This project aims to enhance patient monitoring for long-term conditions using remote monitoring technologies. By leveraging smartphone and wearable devices, it seeks to provide real-time data to improve care efficiency and patient outcomes across various health conditions.
Background:
Long term conditions require monitoring of patients, traditionally conducted in the clinic, to monitor treatment effects, adverse events and disease course.
