CHRONIOUS aims to create a health monitoring platform for chronic disease management, utilizing smart wearable technology to track patient activity and vital signs. The system alerts healthcare providers to abnormal health conditions, enhancing personalized care.
Chronic diseases are those that occur across the whole spectrum of illness, mental health problems and injuries.
